Photo Consent Form.

We must receive approval from all SMU families to post photos on our web site, otherwise we may not allow photos to be placed on this web site altogether including group shots!

For the past 3 years, we posted photos (only) of the students from Band Camp and at every performance here on smupa.com for the enjoyment of our parents and former students. This is a very popular feature of our site. The State of New Jersey now requires that we obtain completed consent forms from all parents to have photos of their students posted on school related web sites.

SMUPA will NOT release/post any “personally identifiable” information such as student names, residential addresses, e-mail address, or phone numbers.

Large digital quality photos we take at all events throughout the year will be available for sale on 2 CD sets (approximately 2,000 photos) before the Christmas season ... they make great Christmas gifts! Some of the photos are also used in our year-end Band Banquet video presentation.

Please make every effort to complete and sign the consent form and return it to the band box in the band room by Wednesday August 22nd (3rd day of band camp) ... at the latest.
